Silica Granulomas a question for Miami Mike or Catamd and a caution for those considering cheap route on enhancements

I know a person in Colombia who has had implants and buttocks injections. Well today she is a very sick puppy. Apparently she used a less than qualified person who injected her buttocks with silicone.

She now has a severe reaction (Silica Granuloma) and high fevers every couple of weeks. This has been fairly constant for the past four years. So can anyone outline treatment options and probable outcomes. I am not expecting good news here.

Let this be a heads up to anyone considering a proceedure.

LPDiver--your firend needs to have a consult with a quality Plastic Surgeon asap. The MD, after a through exam, will make a diagnosis and a plan of treatment. I doubt any reputable doctor would outline a Treatment program with first seeing the patient to ascertain exactly what the problem is and how extensive it is. Should be a heads up to anyone considering this procedure to use a Reputable Surgeon and Clinic. Good Luck to your friend.

Again, this fat transfer/injection you speak of has its drawbacks also if the Patient has this done by a less then an experienced Surgeon. One Hialeah woman I know had this done by a Miami Area Plastic Surgeon and she ended up with vastly unequal backsides. She walked down the street with a Lopsided backside, one side of her butt wa larger then the other. She paid over $8000 for this error and then paid again to correct it. Spend the Extra money in the first place and go to a first class surgeon. Caveat Emptor

She is seeing the best doctor she can afford.

Personally, I expect she will probably die a long slow suffering death.

Hi. Let me tell you about my experience with buttock augmentation and tissue fillers.
First of all let's discuss lipoinjection (e.g. injecting your own fat to make your buttocks bigger). There are three reasons why I do not perform this procedure, here they go:
1. Fat tissue has very little effective blood vessels, and it gets it's blood supply from the overlying skin or the underlying muscle. After fat has been suctioned in a liposuction, it is injured, no matter what technique is used. It has been disrupted and fatty cells are really harmed. When ths surgeon injects the fat into the buttocks, local tissues have to provide oxygen and nutrients for fat to survive. In some cases this does not happens and transferred tissues die, with a high probability of infection and sepsis (life threatening infection that compromises various organs of the body and can cause death).
2. Lipoinjection is done blindly, it involves the injection of a large volume of fat in an emulsified state inside the buttocks. There can be large dilated veins in this area of the body, and if unfortunately the surgeon harms one of this vessels with the tip of the lipoinjection cannula (a large and long needle), fat will suffer embolism, and can go straight to the heart and lungs. If this happens, mortality rates are over 80% and there is NOTHING that can be done for the patient. Most cases of liposuction related deaths in Colombia in the last 5 years are related to massive fat embolisms during lipoinjections.
3. Lipoinjection does NOT offer permanent results. After 6 months the body starts to absorb the injected fat and flaccid buttocks appear.
I do not perform lipoinjection. Never have, never will. It is far too risky and Sociedad Colombiana de Cirugia Plastica has adviced it's members against performing this procedure. If the surgeon decides to take the risk (and many, many do), he or she MUST inform the patient about possible complications, including severe infection, sepsis, embolism, death and permanent disfiguration.
Now about tissue fillers. This has been a subject I am highly interested in because of the massive number of complications related to this procedure. In Colombia there are people who are not even doctors who use them, and there are thousands of aestheticians and cosmetologists who against all laws cause patients irreversible problems using different substances.
Silicone oil has been banned for medical use (except in some cases of eye surgery) in the US for over 40 years, but people in Colombia don't know about that. Silicone oil (totally different from silicone gel usted in breast and buttock implants) inevitably forms granulomas (horrible disfiguring lumps of silicone sorrounded by scar tissue), not only in the place where they were originally injected, but everywhere the lymphatic circulation has taken silicone particles. The worst part is nobody knows why this happens to some people while some people don't react in a severe manner to this substance. Adverse reactions can ocurr anytime (from day one to 25 years later, as reported in several articles in medical literature) after injection. Granulomas are disfiguring and they become infected and swollen for cyclic periods of time, and there is no cure for this disease. If one decides to operate on the site the granuloma has appeared, the patient must be informed about the high risk of recurrence, for as I already explained, silicone oil migrates all over the place, and other particles will indeed form new granulomas. There is no procedure or medication that has proven effective for this condition. In many cases fistulas ocurr, and people have disgusting white foul smelling stuff coming out of various holes in the skin. This does not have any effective treatment either.
Not only silicone oil causes this horrible complication. Medical literature reports similar reactions with collagen, hydroxiapathite, ceramides and the most injected tissue fillers in Colombia : BIOPOLYMERS. Beware of this substances. Cosmetologists don't have a clue what they contain.
In my experience the only safe tissue filler to be used anywhere in the body is Hyaluronic Acid. It is derived from a substance all humans have in their joints and it is not permanent, the body eliminates it after 10 to 18 months. No adverse reactions, granulomas or allergies have been reported so far.
In conclusion, and after a long long post (please don't get bored!!!) the only effective, permanent and safe way to augmentate the buttocks for me is buttock implants. Results are beautiful and safe. In many cases a simple reshaping of the buttock, lower back and thigh area with liposuction is enough to achieve amazing results.
