Like many terms in Colombian slang, it can have more than one meaning.

It is sometimes used to refer to someone dishonest or underhanded particularly in business or personal dealings. From what I have heard this is the most common use in Colombia.

It can also mean mischievious. In this way it is sometimes used as an adjetive. For example "Ella tiene una sonrisa picara" would mean "She has a mischievious smile".

Also, it can be used as a term of endearment, although this seems to be less common in Colombia. In this case it can mean someone who is very capable or talented.
