Marriage by proxy: how?

Can anybody who has experience with marriage by proxy tell me how it is done? What documents do I need, who needs to go, and where? Also, do you know the names or addresses of where my girlfriend can go to get it done? I am a Canadian citizen. Any help would be appreciated.
