How many copies of the DS-156 does my wife need to take to her K3 interview

We have 2 copies, 1 copy which I filled out and sent to her and the 2nd copy which she got from Bank Union, was filled out and stamped by them. Does she need to take my copy or just the bank's copy. Also I dont know if theres is a problem with her DS-156 since the bank gave her a hard time because she was born in Venezuela but has lived in Barranquilla all her life and her mother is Barranquillera. Her passport is Colombian and says Colombiano on it, will this be an issue at the embassy.
