on the g325a the first line, (famliy name) i just use the first last name correct? not her mothers maiden name. the reason i ask is that in colombia they use both names. just want to be certain. If not, would i put her second last name in the "all other names used box"? In addition, right below that there is a box that says husband or wife? If this is my wife's g-325a would i put her info again in that set of boxes? Also on all sections where there is nothing applicable, do i write, not applicable??

This way her name on the USCIS documents matched her passport, cedula, birth certificate etc. I told them what I had done in my cover letter.
