G325A - VERY IMPORTANT - Just Got Married

Should things such as 'calle' and 'transversal' be translated on the G-325a for addresses and employers on my wife's form?
Also, in the "this form is submitted in connection with application for" what should my wife put in this box? "other" petition for alien relative?? I assume i am going to mark other and put "petition for alien relative" no? Please help.
