I'm new to this forum. Does anyone have form OF-169 that
they can e-mail me or where I can download a Colombian
checklist? It would be greatly appreciated. Fab.......

The State Department is the State Department, and a DS-2001 is a DS-2001 no matter which embassy website you get it from. This may be easier than trying to locate OF-169, which is the older form anyway.

rjstuff says on Nov 7, 2005, 07:13: I have an old packet 3 with the OF 169 which is just a declaration page that you fax back to the consulate (along with DS230.) If you want a copy send me a message with your email and I will send it. I tried to load it to my image gallery but it doesn't like it as its not an image!
