BEST WAY TO TRANSLATE A BIRTH CERTIFICATE FROM ENGLISH INTO SPANISH

Hello there beautiful all,
I would like to know if there's a particular way to translate a Birth Certificate into spanish. What I'm trying to say is.....does it have to come up the same way the original Certificate is lay down. With all the boxes and so forth. I would appreciate any help from you guys out there,

I Did The Same... as Utopiacowboy. Both for items used in Colombia and in the United States. Also as Mario says there is no need to try and duplicate all the boxes, seals, etc. Try to follow the general layout of the original document. Be sure and include a certificate of translation. This is the one we use in both English and Spanish and it has never been kicked back.

Example notary seal from the civil registry: Just write name of the notary and below that (seal) (Signature)and the name and address of the notary.

I hereby state and attest I am qualified to translate from the xxxx language to the xxxxx language and certify that the preceding translation(s) are a true and accurate translation from the xxxxx language in the original document.
