you already starting in medellin so i assume that does not come under the three places... around medellin you should visit santa fe de antioquia a lovely colonial village also guatape and the rock of el penol.. there are several other nice villages to visit around Medellin for example El Retiro and Jardin..

you have to visit Cartagena... from there you could make a side trip to Mompox (5hours or so) its a beautiful very historic town on the magdalena river only accessible by boat.. combined with taxi from cartagena..

go to san andres, i was there a month ago for the second time really enjoyed it... buy a package tour with decameron.... its around US$380 each with four nights, flights and unlimited drinks adn food included... you could fly out of medellin and return to cartagena saving you the cost of a flight to cartagena....

from cartagena you can visit the islas de rosario as well...and perhaps the mud volcanoe....

from medellin it might be worht going a few hours south to the coffee zone and taking in some hot pools (santa rosa de cabal) and a visit to a coffee farm... i know a good one if you want a recommendations
