There might be a direct bus but you probably have to change in Medellin. It's probably around 18 hours. Buy tickets at the bus terminal.

Saltador,I took the trip from Cartagena to Manizales with my novia and it was 19 hours,I hated it but it didn't bother my novia one bit but she has made the trip many,many times.Pm me and let me know when you will be in Manizales and maybe I'll be there then,I missed you last time.My novia is 4 months pregnant so I have been spending a lot of time there lately. We bought the tickets at the bus terminal,I don't remember the cost though.

If you choose to take the bus, remember to take a warm jacket with you. Don't know why, but those buses get too cold after about an hour or so and the driver won't let you take anything out of the hold. Took a bus from Medellin to Santa Marta. Let's just say that next time I'll fly or send my regrets.
